Group Practice Info

Thank you for reaching out to Samata Health and indicating that you are a part of a group practice! Below, please find more information regarding the process for your group practice to apply to join Samata Health.

Please complete the two part application process as outlined below:

(1) Please complete our Group Practice Intake Form. This form should be completed by the practice owner or practice administrator.

(2) Please have each of your therapists complete the Samata Health Provider Profile Intake Form. This form can be completed by a practice administrator or by the therapists themselves. Please indicate that you are a part of a group practice when you complete the form (it will be mentioned in a required field).
